Nope. Approval of both houses of Congress is all that's needed.

True ... and it would require a Constitutional amendment to actually begin the process. But to the Democrat/Left, the Constitution can be amended by a mere executive order. Let’s see if the Roberts USSC will then honor their oaths to “preserve, protect and defend” our Consitution from those who’s objective is to destroy it.

For territories. Washington D.C. is prohibited in the Constitution from being a state. If the Constitution matters anymore.

No, not in the case of DC, since DC is specifically designated as a district rather than a state in the Constitution. So to change that designation requires a Constitutional amendment rather than a simple act of Congress.
