Yes. I am well acquainted with that mishap. Unfortunate.

Granted, it was a known flaw in the plane that had to be trained around, but...there are a lot of things that have to be trained around, and we expect people to get those things right.

Especially fighter pilots.

In my other post, I was talking about the book “Bogeys and Bandits” and a despicable woman who tried to paint her instructors for mistakes that were all hers using the “Sex Card” to defame them and hurt their careers.

It has been a long time, but I recall one of the things that could get you washed out of the fighter program was what are called “Blue on Blue” incidents, where in practicing aerial warfare, you shoot down one of your own planes. I believe it is a black mark, and (depending on the circumstances) doing it more than once was enough to get you washed out. I think she had three of them, and after the third one, she was blaming it her male instructor’s treatment of her.

They said her instructor, some Lieutenant Commander, was known for his patience and excellent instruction, and after one of the incidents, he was trying to explain what she did wrong, and she blew him off with an airy response. He persisted in trying to impress on her what she did wrong, to the point this excellent and most reasonable and patient instructors was nearly red with rage because she wasn’t listening.

And I don’t believe she even got washed out for that. It was something even more egregious, IIRC.
