When Franklin D. Roosevelt died on April 12, 1945, his Vice President, Harry S. Truman, was immediately sworn in as President. Truman served until 1949 without a Vice President.

That was before the 25th amendment was passed, which calls for a new vice president whenever that office had been vacated.
