A Mastectomy may not answer her problems unless she loses weight first. The muscle structure could be removed, but with all the surrounding fat still there, the visual effect would be about the same. Just another fatty mass.
She is under the delusion that some specific procedure is going to magically make her happy. No it won’t. Putting too much faith in a needless surgical event.
That much weight is most likely the result of clinical depression and a badly broken spirit.

I read a completely serious article maybe ten years ago about how silicone breast implants were somehow chemically causing depression and suicide. No consideration was given to the theory that women were expecting a perfect life because of big boobs and fount out different.
