I had my employer ask me to tell him if I planned to leave the state and if I do, when I return, I am required to be tested and quarantined for 14 days, unpaid of course.
It is none of their business but it may become an issue. I expect the next step will be to require me to get the vaccine.
Since the vaccine makers are exempt from any liability ask your employer to sign a statement that they will cover your medical bills and living expenses for the rest of your life if you can no longer work due to vaccine caused disability.
Also, if you die from it, pay the cost of burial expense and death benefit for spouse.

Over 95% of the workforce at my employer is currently working from home every day and it not permitted to enter the building at ALL.
Out of the remaining 5% who have access only a handful are actually in the office every day.
I was reassigned in my job after 5 years to an entirely different department and task and have to be there every day (and my old job had been transferred to Asia where there is no lockdown).
I was able to do my previous job from home during the lockdown.
It is unfair of the company to put me at personal risk of Covid exposure (a coworker tested positive this week) for no extra pay, having to pick up commuting costs while most co-workers do not, and certainly I should not be mandated to get an vaccine when again 95% of the employer’s workforce would not.
