In my experience, outside of the professor/student relationship, a PhD insisting to be called “Doctor” is usually a red flag. Pretty good sign that you’re dealing with someone mediocre or worse with an inflated sense of worth.

True that .... or even an intellectual Napoleonic complex.
The other place it is commonly used - certain litigation where it is important to keep addressing the witness as “Dr. X Bert Witness” to subconsciously imply to the jury just how smart the expert is, ‘specially when the good doctor has a little difficulty “’splaining tings” in non-expert language.
Well that is until a non-credentialed expert like “Miss Vito” shows up, as in My Cousin Vinnie - then it backfires, and all bets are off.
