Often there is a sensation of needing to urinate when there is a catheter — this is normal

It sounds like he is having bladder spasm. Ask the physician if a B&O suppository would help — this specifically is the treatment of bladder spasm. However, beware, you will get a lot of advice including mine — consider carefully that you have asked a health question on an internet where qualifications cannot be assured.

head nurse just told my mom they gave him anti-spasm medicine maybe 6 hours ago...wouldn’t it start working by now?

That was my suggestion, B&O suppository. Also have them make sure the catheter is in proper position. Sometimes it isn’t fully inserted and the balloon is still n the urethra. This is very uncomfortable.
