Iceberg warnings but full speed ahead!
Too easy. Titanic was sailed into upper latitudes meaning known ice fields to speed up her arrival in New York. She was sailing at 22 1/2 knots, above her normal cruising speed.
The lookouts who spotted the iceberg too late had no binoculars because these were locked in a chest in the crow’s nest. The holder of the key had missed sailing on the Titanic.
That key just sold at auction for $500,000.
Oh yes, and the loss of all those influential plutocrats hastened the passing of the Federal income tax just a year later.
And the lookouts’ alarm was delayed because they were too busy ogling Kate Winslet.

Iceberg warnings but full speed ahead!
Ive heard that the theory at that time was to spend as little time as possible in the danger area, hence full speed. Which makes a certain sense, but has its own dangers - as the Titanic discovered.
