Trump's speech today shed light on what he was talking about here. About 24 minutes into C-SPAN's recording, he's talking about his executive order on cutting drug costs, and he mentions that he's going to make some "very rich" people behind the drug companies angry. He says he doesn't know who they are, he just knows they've been making a lot of money.
