Go to the thread and click on the the word letter; its a link to the form. Funny it says, Signatories have been vetted to the best of our ability. ha!

Yes. That’s where I first discovered it. The link and document submission process was still functioning 5 minutes ago. The form asked for your name, city, length of service, highest position attained, administrations served under, and maybe one or two other things but not much more.
