Are the Russians pushing the equipment to, or past the design limits? Or defective components/ poor workmanship?

Probably a little of both. Russian technology is dark age stuff, and there's not much incentive to improve it, at least for consumer goods. For military gear, you'd think the incentive would be there, especially with Putin at the helm. But decades of socialist ennui (and drunkenness) have created a workforce that seems to not give a tinker's borscht about quality.
Very dangerous in situations where failure = catastrophe. Like on a submarine. Or a spacecraft.
