With all of the exceptions to the quarantine, how can a quarantine actually be effective?

Minors excluded, only aliens quarantined? Dozens of other groups? (What? A US citizen won't get infected in China because they are a US citizen? A child won't get infected or won't be a carrier just because she is a child of a US citizen?)
