I Took some time to research this.

In 1991 Caroline Cossey appeared in Playboy. Caroline was a boy born with a variant of Klinefelter Syndrome where a male is born with 1 or more extra X chromosomes. Cossey has the XXXY variant, usually called Klinefelter 48.
Cossey also appeared in the James Bond movie “For Your Eyes Only”.
