I used to climb up sheer quarry walls at that age -- but yeah, I would think that a harness would be required on a rock wall. The main risk at a trampoline park would be neck injuries from people trying flips or bouncing off the trampoline and onto the side platform. Those risks are inherent in using a trampoline. But I though harnesses were standard on walls, because what comes up must come down. Even if people can make the climb safely, the descent is trickier. And people do a lot of precarious tip of the toes leaning and stretching to try to grab the next handhold, which makes for potentially awkward falls.

My guess is that a teenage employee didn't do his job.

I used to climb with the Colorado Montain CLub. My buddy was a single guy and over time I noticed that he too frequently was not paying attention to me when I was on belay. Sort of soured me on the experience.

“My guess is that a teenage employee didn’t do his job.”

A teenager should never be in charge of a child’s life.
My guess, an adult F’d up.

Me too but I have a deep pool of water under me should I fall. Then I jumped into it after the climb

Climbed trees too but most had lots of branches..
