Medical notes written after every doctor’s hospital visits/procedures are (or were) not permitted to have any lines skipped. I find it strange that these notes have such a variety of lines skipped (or none) between entries.

In my opinion notes are written for the writer in order to read easy..and skipping lines helps when reading..notes are a personal reflection. There are no guide lines for notes. But then maybe I don't know exactly what your point is..lol
