I fly Phoenix AZ - Lihue HI often. This works for me:

Half hour before airborne 10 mg Diazepam.
Business class is better than first, Imo.
Foam ear plugs.
Bose over the ear noise canceling headphones.
Blindfold.
Large beach towel serving as a blanket draped over my head.
Seat belt fastened and visible.
Forty minutes after airborne 10 mg Ambien.

Identical to my routine except I pounded a good, healthy swig of Bacardi 151 instead of Ambien.
