Spain built a new class of submarines.

Turns out they didn’t have enough buoyancy. They could dive, but not come back up again properly.

Back to the drawing board & the docks.

They added length to the submarines, so there would be more displacement.

THEN they found out the new, longer subs DID NOT FIT into the existing docks.

Same engineers as working in Seattle???

Having worked for thirty years in the military industrial complex I can tell you what happened. The engineers raised a flag early on. But the people at the top were focused on award fees and bonuses and refused to do anything that would affect schedule. By the time the problem was discovered, the top level people who allowed it to happen had retired (much richer for not avoiding the problem) or had moved on to bigger and better jobs somewhere else.
