Have there ever been stampedes at the Vatican when the faithful gather there?
Is there something about Islam or the geography of Mecca, the heat and tough weather of Mecca, which causes this? There have been many stampedes and mass deaths there in the past. The stampedes seem to happen regularly though not every year.

The hajj only occurs once a year. There is something called the lesser pilgrimage where one can visit Mecca any other time but that does not meet the requirement.
I wonder which one would have the greatest affect?
You can’t enter Mecca if you are not a Muslim. They check your IDs at the city limits. Historically, there have been many mass tragedies during the Haj. The logistics have improved, but you can’t get that many people together in such a relatively small area without having problems.

How does Saudi Arabia handle the visas and logistics of 2 million people at any given time?
I assume travel visas are short-term. I also assume these tourists are not allowed to travel around the country at will. I also assume they are treated quite harshly if they overstay their visas or commit any crimes when in Saudi Arabia.
It is difficult to enter the Kingdom unless someone sponsors you, primarily an employer. There is no real tourism in Saudi Arabia, but there is a nascent attempt to bring in tourists to selected areas, like the Red Sea.
