Here is how the airplanes were arranged when the incident took place:
McCain's airplane is labeled #416. Note that it's exhaust, if any, is pointed over the ocean. The plane which fired the Zuni is #110. The rocket hit #405, piloted by LCDR Fred D. White, knocking off a fuel tank and starting a conflagration. McCain scrambled over his plane's nose and jumped off its refueling probe. White also escaped from his plane, but was killed when one of the bombs cooked off.
The Zuni fired when the pilot of #110 switched his plane from external to internal power. The switchover caused a power surge, firing the rocket. Two separate mechanisms to prevent such an accident were bypassed: (1) a safety pin had been blown out by high winds and (2) a "pigtail" connection to the rocket pod, supposed to be plugged in only when the airplane was on the catapult, had been plugged in early in an attempt to speed up the launching process.

I see your diagram, but you fail to take into account that it was windy that day, and the exhaust drifted from McCain’s engine to the missile that McCain indirectly but maliciously touched off.
