You make excellent points. I wish doctors understood that diabetes is almost 100% curable if it hasn’t done too much ancillary damage. (some do, but very few). I fear that the huge money made from diabetes treatments leads to little motivation by pharma/medical system to implement a dietary approach. The treatment regimen today is barbaric.

My Wife (neurologist) and I have been looking at a very rare genetic disorder that leads to rapid iron accumulation in children to young adults. It is both a horrific and tragic disorder that leads to death in the late teens (if not earlier). From the research, we were searching for other pathways besides the Pyruvate Cycle to directly "feed" the brain. The disruption of the pyruvate cycle in these children is similar to the effects of long term metabolic syndrome (except they were starving to death). The pyruvate cycle (sugars) is a 27 step metabolic process. Any disruption in one of those steps leads to the beginning of metabolic damage to the brain. We found the secondary pathway (carnitine shuttle) was the key to feeding the brain and avoiding issues with insulin disregulation.

Medium Chain Fatty Acids (MCFA) follow only a two step process and cross the BBB with this secondary pathway. The steps are fairly simple with digestion and routing from the portal vein to the liver and then the bloodstream. From there, the MCFAs are available for the carnitine shuttle.

MCFAs can be found in foods such as coconut and palm oils. Coconut is not complete MCFA, but is adequate. We were especially interested in Caprylic Acid (surprisingly available on Amazon). The brain utilizes approximately 20% of human energy. Without available energy (Insulin Resistance and disruption of the Pyruvate Cycle) damage to the BBB and brain occur. We believe this can be stabilized, however are uncertain how much the damage can be reversed. There has been some promising research that indicates the BBB begins to repair itself on a VLC diet rich in MCFAs. Further, a couple of affordable supplements have also been shown to repair the BBB and remove the accumulated iron in the brain. The treatment is only viable longterm if the insulin resistance is reversed.
