I think your suspicions are correct. Look at Hillary’s face, it looks stretched. The size of her head looks like it isn’t at the same scale as the rest of the photo. There is also something going on with the dark on the left side of her head.

I looked up Hillary's pets. The dog in the photo does look like the one they call Maisie. I suspect the picture is faked by placing her face on either Chelsea or a dog walker. Lots of pictures online with their dogs being walked.

Here is a what looks like a real picture of her walking one of her dogs. Compare the cankles, legs general body shape. Not likely the same person in both photos.

It looks like someone cut and pasted her head onto that picture.
