One of my distant relatives has a similar cancer.
They operated for appendicitis and found it.
He’s on a chemo regimen with a poor prognosis, although he’s significantly older (67) than Scott.

Appendix cancer is extremely rare, affecting an estimated 600 to 1,000 Americans each year. Its mostly genetic and HSS very little to do with lifestyle choices
