Keep in mind that the pilots set the air pressure inside the aircraft to be equivalent to about 6,000 to 8,000 feet above sea level.

and... with the low cabin pressure... every O2 mask failed to drop? How long to masks take to drop? Where is the pressure sensor that triggers the mask drop? How long does oxygen start to flow to the masks - including the pilots' masks? If there was an rapid decompression, the entire oxygen mask system may not have even deployed, or not delivered oxygen fast enough to do any good. If the mask sensor is anywhere near where a hull break occurred, the entire system could have been rendered useless. For as many theories there are of nefarious actions being the cause, there are at least as many explanations of good old systems failures being the cause.
