Have you never hit control-W instead of shift-W? Control-W instantly kills whatever page your have open.

Ctrl + Shift + T opens the last-closed tab, you can do this multiple times. (Assuming you're talking Firefox.)

A few minutes searching around on the ctrl-w question leaves me with the impression that there’s no easy way to disable that key pair.
I have to tell you, I’ve been using Windows and Internet Explorer for almost twenty years, and I never even knew about ctrl-W until reading your post.
On the one hand, it seems like a dangerous shortcut to have put into IE. On the other hand, it’s hard for me to see how you could hit that particular key pair by accident, but everyone’s keyboard habits are different.
When I want to kill one tabbed view without killing them all, I use ctrl-F4, which seems to be the same as ctrl-w.
How do you make this mistake? Do you have a number of browser windows open at the same time, each with a number of tabbed views, decide you want to close one thinking it has focus, hit ctrl-w, and then realize that the tab that had focus is not the one you thought had focus?
