Usually ‘mishaps’ with Orca versus human are attributed to wetsuits making humans look like seals. That injury tends to be an orca ramming a surfboard or swimmer.
There was a captive orca who killed a trainer and swam with her body in his mouth and refused to release her until they got him onto a platform and raised him out of the water. However - any captive animal is bound to behave with extremes. Also a captive orca is believed to have breached and landed intentionally on a trainer - again believed to be aggression.

From the reports I’ve seen, most (if not all) killer whale attacks on humans involve captive whales.
