Traditional surgeons perform the procedure in the bush, sometimes with unsterilised instruments or lacking in technique.
Botched circumcisions leading to penis amputations and deaths are an annual tragedy.

My father in law had to be circumcised in his mid 70’s also for medical reasons. He had to wear a mumu for two weeks.

Might want to consider building a couple of temples, inviting Jews from Muslim countries to come live there. Wouldn’t hurt to also bring in some Israeli paramilitary to watch after them and help the locals.
