On a pure libertarian basis you are correct.

It’s the ones that don’t get killed and run up multi-million dollar repair bills and lifetime custodial care at taxpayer expense that are a financial issue.

There is because there are parasitic laws that force hospitals to treat those who cannot afford their treatments. If you do not wear seatbelts and cannot afford medical care for a vehicular accident, then you should not be behind a vehicle. The laws protect the societal parasites and punish those who should be protected. To address the effect of the pre-existing parasitic laws, more parasitic laws are added.
