So butthurt fanfic writer takes the manuscript, changes the names, adds a few extra paragraphs maybe, and submits it to a publisher.

Publisher bought it.

I am assuming you have to be able to write. When Geofrey Jenkins wrote the James Bond novel "Per Fine Ounce" it was rejected due to lack of quality.
