I have a friend who has taken a prescription medication for a couple of years. He was just told that he is now too old and either medicare or his insurance (not sure which, he has both) will not pay for it.

Yep.
Probably medicare.
When I had private insurance they paid the synthetic drug cost and I picked up the difference. I have a weird thyroid issue and synthetics don’t work.
I found that fair based on the explanation.
When I was forced off my private plan and onto medicare, they said they would pay nothing. I called and argued with them about it, and they said they would would for six months, after that I needed to change the meds. My doctor wrote a letter of explanation - nope.
I feel for your friend, fortunately places like Sam’s Club, WalMart or Target have very low perception costs.
