I’m guessing you didn’t watch last night, either. Shane ferreted off Randall from the barn to extract info as to Randall’s camp, then snapped his neck, killing him. He did not brainpan him, thus, he didn’t anticipate Randall rising once killed, which he did.

Rick killed Shane with a knife, then turned his back on him as Carl ran up. Shane was in the process of turning, but only Carl could see it as Rick was facing Carl.
