I think it was a culmination of the Grand Jury presentment coupled with enough investigation into the victims and counts/charges

I saw one article that says Sandusky has known about some of the more recent charges going back 3 yrs or almost 3 yrs. So they've been working on this for a while now.

Yes. In 2009 the parents of a 10 year old made a molestation complaint to police. Since then they've been digging.
