The server (waiter or waitress) and the cook/chef are two different job titles. One really doesn’t have anything to do with the other. If I have an issue with the food, I will send it back. I think I have NOT tipped twice in my life. Both times, we had a waitress who was beyond insulting and rude. I will say that it is a very rare occurrence.
This is true. I always make that distinction, unless the meal sucks, and the waitress doesn't seem sufficiently motivated to 'make it right'. I also tip on the PRE-tax amount of the check. :p
