Well, not according to the official investigation. THEY DON'T KNOW EXACTLY WHAT HAPPENED but focused on a power surge in the changing from external to onboard (the aircraft) power. There seems to be an equipment failure history and procedures that weren't exactly followed. I don't think that there was a accusation that McCain was at fault (except by McCain haters of course). A MISSILE FROM HIS F4 DID FIRE PREMATURELY AND HIT ANOTHER AIRCRAFT. There was a major investigation that resulted in many changes in the staging procedures, fire fighting equipment and damage control procedures on board carriers. The lesson learned from that accident is still taught today in boot camp and various Damage control schools.
