The insurer has a contract which should tell what is covered and what isn't. Just as I don't expect my homeowner's insurance to pay for replacing my garage door when it breaks from old age because the insurance contract says that maintenance isn't covered, I should be able to read my health insurance contract to see what is covered. Also, whether insurance covers a procedure or not does not prevent me from paying for it myself, even if it would extend my life and make me "unprofitable" for the insurance company (you were supposed to die years ago!)
The government program will have no contract and no qualms about preventing me from paying for my own treatment which could make me more expensive in the long term. See the British NHS which likes refusing to pay for any future coverage if you get lifesaving treatment outside of the plan. Also, I dare you to find any legally enforceable contract from the government detailing what is covered now and will be covered in the future under Obamacare. The Nestor decision said that Social Security isn't a contract and that Congress can change its terms at its whim; Obamacare will be the same.
Sure seems like they are planning to implement the British system here.
