Hmm, no on all counts. Starting with the last.

Most likely the posted Certification of Live Birth that was posted was just made up of whole cloth, in a graphics program like Photoshop, not "copied" from any other COLB. But in any event, that is not the "Long Form" the long form from the early 60s looks like this:

As you can see they are two different forms entirely.

All reports indicate she was born in Indonesia.

Never heard of S. A. returing to Hawaii for Maya's birth. Has anyone else? According to Obama.net, and many other sources/ She was born on August 15th, 1970 in Jakarta, Indonesia.
