I agree. It's not a sixth toe, it's an enlargement at the base of her little toe. Probably the fifth metatarsal - a/k/a 'tailor's bunion'. So called because tailors sat cross-legged on a bench with the outside of the foot pressed against the floor, which tended to cause this condition.
If somebody has a tendency to flat feet and they also wear tight, high-heeled shoes, the head of the metatarsal will flare out. Sometimes the joint gets so far out it looks like a sixth toe.

It's a very painful condition, that's probably why she always looks so unhappy.
