You obviously have not looked at baby footprints. Go to google.com, type in “baby footprints” and enter, then select “Images” on the top, in the first page alone, there are 3 examples. I checked online and the first pad is for the Great toe, the second pad is for the middle 3 toes, and the third pad is for the little toe. The pads are for balance, forward traction, and gripping. Very common. There are exercises to increase the strength of the toes and those pads as well for gymnastics, track and field, etc.
