You may not have been born in a hospital but is there a provision for “midwife” on your form?

There is on mine and many other I have seen including the one for my friend who is 99.

I was born in January (19 and 41) and it was April before my parents registered me.. I wonder. No. There were no other choices, I suppose. I think most states allow up to a year (for the parents to make up their minds?).

There was a place for a witness (attendant) to sign labeled, physician, midwife or other.

In my case the attendant was a doctor.

What if "other" signed it and was paid (or a friend agreed) to say s/he witnessed the birth but I was actually born.. who knows where.
