2003-09-03 off the Cubs' Felix Sanchez
2006-07-28 off the Nats' Roy Corcoran
That was the 3rd Red Sox postseason grand slam, joining:
Troy O'Leary's in Game 5 of the 1999 ALDS.
Johnny Damon's in Game 7 of the 2004 ALCS.
The Red Sox won both of the above games.

Alex Cora has been interviewed on the radio pre-game show before every ALCS game. He said that Youk would be tonight’s hero. Cora has eerily right in just about all his analysis so far.
