You are mistaken.

(a) It is not simple mutilation unless you do not desire said surgery,
(b) It is always coupled with hormone treatments. In cases of male to female estrogen brings changes to body structure, redistribution of weight and muscle mass, together with changes to skin and other physical aspects. Where the patient is Female to Male, testosterone helps deepen the voice, bulk muscle, promote facial hair growth among other things.
(c) Sex can be reassigned. Gender cannot. There is a very real difference between these things. The sex is your genitals, your gender is your identity.

I suggest you read Brain Sex by Anne Moir Ph.D and David Jessel (review at http://www.rightgrrl.com/bookreviews/brainsex.html).

It's easy to be conservative, the trick is to be an informed one.
