It was originally done by Bud Abbot and Lou Costello. You can see the entire bit in "The Naughty Nineties" which either Blockbuster or Hollywood Video has carried in the very recent past. I do it with Toby and Mr. Bates because those were the two characters in the Toby Show in Branson, Mo. where I used to see it done.

Toby: (suspicious look)You're the manager?
Mr. Bates: That's right.
Toby: You know the fella's names?
Mr. Bates: Well, I should.
Toby: Then Who's on first?
Mr. Bates: Yes.
Toby: I mean the fella's name.
Mr. Bates: Who!
Toby: (frustrated) The first baseman!
Mr. Bates: Who!
Toby: (angrily) The guy playing first base!
Mr. Bates: Who's on first!
Toby: Look, What's the name of the first baseman!
Mr. Bates: No, What's the name of the second baseman!
Toby: I Don't Know!
Mr. Bates: He's on third. We weren't talking about him!
Toby: (screaming) How'd I get on third base?
