A man that heavy will have a huge fat pad completely engulfing any of his male "equipment," which will appear to be a dimple, like a second navel, and which will stink to high heaven.

Is it hard to um.. keep clean or something? I dont understand why it would stink.

Every grossly fat person I have ever known, male or female, has had yeast infection of all the skin folds EVERYWHERE. Some of them come into the ER, and stink up the entire place. It isn't dirt, it's rot.
