You don't see someone dirty getting legitimate sex change assistance because the patient has to undergo strict psychological counseling before they can even start. Therefore if he's physically dirty, he's gotten the hormones from somewhere else, like black market or stealing them himself.
There's a recent murder case of a married couple in Mass. She was an M.D. and he was something tangential to medicine that I forget. 16-20 years into their marriage he killed her. I guess they started out okay but he got nasty and slapping turned to beating. He was wearing her clothes and forcing her to provide him with vast quantities of hormones. By the time he killed her he had little juvenile boobies and a pageboy. He was an ugly bastard no matter what sex he was. I wish I could remember his name; he's probably having a tough time in jail...which he deserves. He killed her with a shotgun in front of their kids.

It is hard for me to believe a doctor would authorize letting that particular man get a sex change. But when you think about it, it is hard to believe any doctor in his or her right mind would authorize anyone to have a sex change (except for those born with both organs, but that is another issue.) If someone is confused, a doctor should be getting them emotional help for their emotional issues!
Too bad the woman doctor didn't report him to the police, take their kids, and walk out on her abusive hubby. Abuse can be a progessive degeneration for the victim too. The abuser gets inside the head of the victim first, to be certain he/she has continued access to the victim and to make sure the victim won't rat 'em out.
