The patient does not realize he is making things up. He proceeds as if he believes his story.
I think Biden has gotten to the point where he is untethered to reality, says whatever he thinks will sound right, and ends up believing his own fictions. I know Biden has been a fundamentally dishonest person his entire life, but this is something different.
