I’ll take his denial as meaning that there was a third shot just that it didn’t cause the death.

IOW, it’s his opinion unless he knows for sure that she never got the booster shot. If he had said that she never got the booster, therefore her death could not have been related to it, that would have been a different matter. But he didn’t say that.
