I assume this has to do with the U.S. Public Health Service. I understand why it is a uniformed service and has a military structure, but I don’t see how this political appointment equates to a real general or flag officer who has spent a career working up to that grade. What real general or admiral starts off with nothing and then suddenly has four stars on his or her shoulders?
