He probably waited too late. Once covid gets serious enough for hospitalization - Ivermectin or MABS won’t help.

Hospitals in TN are full and there isn’t sufficient staff to give them. Reality isn’t also pretty.

His O2 level may have been too low. Cutoff level is around 93%. Once it drops outcomes are believed to be worse with MABs than without them.
