You are a doctor for life. But you must be registered to Practice.
As a matter of courtesy you could address him as Dr. Paul but technically not if you no longer have a doctor's license.......
Just like past politicians are politely addressed as "Mr. Speaker" (Newt Gingrich) or "Mr. President", any of the priors.......
