If it were the design, the AK would not be the most produced firearm of all time.
Looking at the slow motion, on the kaboom round the charging handle didn’t move at all, which means no pressure at the gas block. So either the bolt didn’t fully lockup, or the bolt locking lugs or barrel locking recesses failed, resulting in the bolt being driven backwards, leaving the bolt carrier (and its attached charging handle) behind.
