Can someone tell me what is going on with the book in this coughing episode around the one minute mark.

Why is someone handing her a book while she is in the middle of a coughing episode. there is something in between the pages. Was she on a commercial plane and this was a dumb citizen wanting an autograph?

This makes no sense to me. She then hurries off to her handler in the back who shuts the curtains.
